The Quick Sensory Assessment is a standalone sensory questionnaire scored and summarized by a licensed occupational therapist. It's designed for anyone who wants a clearer picture of how their nervous system processes the world — without committing to a full clinical evaluation or ongoing therapy.
You complete the questionnaire online. We score it, summarize the results, and send you a basic score report within 1–3 business days of questionnaire completion. What you do with it is entirely up to you.

Receive your questionnaire
You'll receive the sensory questionnaire via email within 1 business day of your intake form and payment being received. Complete it at your own pace — it typically takes 15–20 minutes.
Get your results
Your basic score report will be sent to your email within 1–3 business days of questionnaire completion. Results are yours to keep, share with other providers, or simply explore on your own.
